Filling out paperwork before your appointment will speed up the process of getting you started.  It is recommended that all our new and returning patients print out our current paperwork and have it ready upon arrival.

Records Release Form

If you need to transfer your records, you will need to fill out a Records Release.  There is no charge for an electronic transfer of records (Fax / eMail), however, If you require a paper copy of your medical records there is a fee of $30.00 + Shipping.

• LJCC Senior Express - If you are a senior citizen the LJCC can help.  LJCC Senior Express is a Transportation Program for Seniors available through the Florence Riford Transportation Fund managed by The San Diego Foundation.  For more information click here.
